Animal Eye Clinic will provide screening examinations for:
|

Certified dogs from the following Service Dog groups: ~ guide dogs ~ handicapped assistance dogs ~ detection dogs ~ search and rescue dogs

Exams are limited to those dogs currently in active service

Dogs in training, or retired service dogs to not qualify.
Service Dogs must have been formally trained and/or have certification
or paperwork attesting to their training through a national, regional
or local organization.

In order to receive an examination:
Log onto www.acvoeyeexam.org.
Owners/agents for the dog must complete the online registration form.
Once registered, you will be provided with contact information for a participating
ophthalmologist in your area.
After registeration, you must contact the ophthalmologist to schedule an
appointment.
The registration number assigned to the service dog must be provided to
the clinic.

Will the pet wellness report be given this year?
|
No, this event is restricted to ocular examination.
|

What if the service dog is diagnosed with an ocular malady and requires services beyond eye screening exams?
|
The complimentary exam is for screening purposes only.
Should an ocular disease be diagnosed, or further diagnostic work-up
be indicated; these would all be at our normal billable rates. Animal Eye Clinic
staff would notify the handler if billable services are suggested time of the exam.
