This form is required for all Initial Examinations. The initial examination includes a Schirmer Tear Test (for dry eye), Tonometry (for glaucoma and ocular inflammation) and Fluorescein Stain (for corneal diseases). These diagnostic tests are included in the cost of the examination and are considered necessary for each patient.
If these forms are filled out in advance, but someone other than the owner of the patient is attending the examination, prior payment arrangements must be made with Animal Eye Clinic staff. The person attending the examination must be able to make decisions regarding medical treatment, or be able to reach the patient's owner in a timely fashion.
Patients scheduled as a drop off, or those being brought in by someone other than the owner are required to fill out an accurate history. All sections of this form must be completed.
Patients scheduled for a drop off examination are required to have all medications they are taking left with them. If necessary, our staff will medicate the patients while they are waiting for examination. Also, it is important for Dr. Paulsen to review medications and alter treatment schedules if indicated.
Patients that are brought to Animal Eye Clinic by someone other than the owner should have all medications with them as well. It is crucial that Dr. Paulsen be aware of all medications that the patient is currently taking. Some medications interact poorly and the proper precautions are vital.
